The client — a mid-size US healthcare provider with 400+ physicians across 35 locations — was running patient outreach, referral tracking, and marketing campaigns on three separate systems that didn't talk to each other. Staff spent 12+ hours per week manually reconciling data between systems. HIPAA compliance required stringent data handling, and the existing systems had no unified audit trail. Patient engagement scores had dropped 22% year-over-year as communications became increasingly fragmented and impersonal. The IT team had attempted a self-managed migration twice before, both ending in rollback due to data integrity issues.
Our Approach
We assembled a 6-person offshore team: 2 Salesforce Health Cloud architects, 2 developers, 1 data migration specialist, and 1 QA engineer. Phase 1 (weeks 1-3): Comprehensive audit of all three legacy systems — mapping 147 data objects, 23 automated workflows, and 89 integration points. Built a unified data model in Salesforce Health Cloud. Phase 2 (weeks 4-8): Iterative migration of 2.3 million patient records with a custom ETL pipeline featuring real-time validation, deduplication (eliminated 34,000 duplicates), and HIPAA-compliant data handling with full encryption at rest and in transit. Phase 3 (weeks 9-12): Built custom Lightning components for referral management, automated patient journey workflows using Salesforce Flow, and integrated with the client's Epic EHR system via HL7 FHIR APIs. Phase 4 (weeks 13-14): User acceptance testing across all 35 locations, staff training for 200+ users, and parallel running with the legacy system before cutover.
