The Challenge

The client, a 500-bed regional hospital network serving over 200,000 patients annually, was struggling with a 15-year-old on-premise CRM that had become a major bottleneck. Patient records were siloed across three different systems, referral processing took an average of 72 hours, and clinical staff spent up to 40% of their time on manual data entry rather than patient care.

The existing system lacked HIPAA-compliant audit trails, had no patient self-service capabilities, and couldn't integrate with their newly adopted EHR platform. With patient satisfaction scores declining and compliance audit deadlines approaching, they needed a modern, cloud-based solution — fast.

Our Approach

We assembled a 6-member cross-functional team from our India delivery center: 2 certified Salesforce Health Cloud developers, 1 MuleSoft integration specialist, 1 Lightning Web Component (LWC) expert, 1 QA engineer, and 1 project coordinator.

Our approach followed a phased methodology:

**Phase 1 – Discovery & Architecture:** We conducted a 2-week deep-dive into their existing workflows, data models, and compliance requirements. We mapped 47 unique patient journey touchpoints and designed a unified data architecture on Health Cloud.

**Phase 2 – Core Migration:** Using MuleSoft Anypoint, we built real-time bidirectional sync between the legacy CRM, the new EHR system, and Salesforce. We migrated 1.2 million patient records with a custom ETL pipeline that included data deduplication and validation.

**Phase 3 – Custom Development:** We built 12 custom Lightning Web Components for the clinical dashboard, including a referral management module, patient timeline view, and care plan tracker. All components were designed mobile-first for iPad use during rounds.

**Phase 4 – Security & Compliance:** Implemented field-level encryption, comprehensive audit logging, role-based access controls, and automated HIPAA compliance checks. Passed third-party security audit on first attempt.

Key Outcomes

40% reduction in patient data entry time
60% faster referral processing
99.8% data migration accuracy
$250K annual cost savings

The Results

The migration was completed 2 weeks ahead of schedule and $50K under budget. Within the first 90 days post-launch:

Clinical staff reported a 40% reduction in data entry time, freeing up an estimated 2,400 hours per month for patient care. Referral processing time dropped from 72 hours to under 4 hours — a 95% improvement. The self-service patient portal saw 68% adoption in the first month, reducing front-desk call volume by 35%.

The hospital network passed their HIPAA compliance audit with zero findings and has since expanded the engagement to Phase 2, adding AI-powered patient risk scoring and predictive analytics dashboards.

"Offshore1st delivered what our previous vendor couldn't in two years. Their Salesforce team understood healthcare workflows from day one, and the quality of their Lightning components rivals anything we've seen from US-based consultancies — at a fraction of the cost."
